Kubasiak, Jerome F. Lovingly called Sam. God knew Sam was getting tired and so He sent his angels to bring him home to Heaven. Sam passed away peacefully surrounded by his loving family after a long battle with lung disease on Saturday October 11, 2008 at the age of 82. Beloved husband of Janet (formerly Rose)(nee Kmet). Loving father of Renee (Joe) Foti, Pat (dear friend Debbie), Sue Guyant, Pam (Bob) Grzybowski, Mary Beth (Jim) Koski and Kathy (Ed) Sparks. Dear brother of Connie (Jerry) Lemanczyk. Brother-in-law of Bob (Sandy) Schubert. Further survived by 11 grandchildren, 9 great grandchildren, other relatives and friends. Memorial visitation Thursday 1-4 PM at the Max A. Sass & Sons Greenridge Chapel. Memorial service Thursday 4 PM. Sam's family wishes to express their gratitude to Wheaton Franciscan Home and Hospice for their compassion and care. In lieu of flowers memorials to the American Lung Association or the American Cancer Society. Sam worked at GE Medical for almost 40 years as a tool grinder and proudly served his country in the Navy. Sam loved to golf and always had to rely on dear friends to help him find his golf balls.
