Kubiak (Mongan), Helen M. (nee Hornak) Born to Eternal Life on March 11, 2011 at the age of 93. Beloved wife of the late Henry Kubiak and the late Tom Mongan. Loving mom of the late Larry (Mary Lynn), Kenny (Diane) and Mary Ann (Wayne) Murphy. Proud grandmother of Linda, Laurie, Kelly, James and Paul. Dear sister of Frank (Lynn) Hornak and Danny (the late Margie) Hornak. Sister-in-law of Ceil and Margie Hornak. Further survived by 8 great grandchildren, step grandchildren, nieces, nephews, other relatives and friends. Visitation Tuesday 4-6 pm with a funeral service at 6 pm at the Max A. Sass & Sons Funeral Home. Private emtombment. Helen’s family wishes to express their heartfelt gratitude to the staff at Franciscan Villa for the compassionate care.
